Goodbye, Montreal !!

Today, I went back from Montreal to Japan.

There was a happening in my first flight.
It was delay one hour, because the plane have to change its broken tire. My flight schedule was tight. It was only 75 minutes for my transit in my schedule. Therefore, it remain 15 minutes to transit.
In the airplane, there are other passengers who go to same destination, Narita.
The staff from the airplane company guided us. we have to rush to international depart port. Fortunately, I can ride the second plane. My stay time in Toronto airport was only 15 minutes.

One of my attending doctor told us that we have to keep our mind open, because medicine is always adventure.
I thought there are always happening in life.

Totally, I transit 8 times, subway, bus, and airplane to go back my hometown.

Cirque du Soleil, Totem




Today was my final day in Montreal.

The hometown of Cirque du Soleil is Montreal. Therefore, Montreal is the first place to start new performance.

The cheapest sheet is even burden for a student. I want not to save the cost for those investment.
My experience to watch circus was only Bolshoi circus for a long ago.

Today was Sunday. So there were so many family groups. I couldn't find any extra available seat. I thought the distance from stage was near from my seat.

It was so exciting for me.
Their physical preform was high quality. They used many stage effects to show their performance more effectively.
It was art, circus, and musical.
Real performance and real music has risk to fail. Audience can feel the player's tension and breathing. It lead audience and the player together.

I want to check it in Tokyo too.

McGill大学感染症内科における実習の報告



2010/4/5~2010/4/30にかけてカナダ、モントリオールのMcGill大学感染症内科にて実習させていただいたので、ご報告させていただきます。
1ヶ月間、1つの病院にて学ばせていただいたので、あくまでカナダ全般またはケベック州全般について言及しているわけではないこと、また日本での経験も医学生として筑波大学にて経験したことを前提に考察していることを最初に断らせていただきます。

ケベック州はフランス語圏のため、日常生活はフランス語を使っていますが、McGill大学は英語系の大学のため、チャート(カルテに相当)は英語で記載されます。フランス語での会話を好む患者さんに対しては、他のスタッフの方や隣の患者さんに通訳をお願いすることで対処しました。実際、自分も挨拶と自己紹介しかフランス語ができない程度でした。

自分の場合は、公募でアプライをしました。カナダは米国に比べて比較的海外の医学生を受け入れています。大学からの受け入れが確定した後は、在日カナダ大使館からの一時滞在者査証の手続きがあります。

 大学付属の病院というものはなく、大学といくつかの教育病院が提携しレジデントや医学生を受け入れていました。自分はその中のRoyal Victoria Hospital (RVH)という病院にて研鑽を積ませていただきました。伺ったところでは病院間では、大きな違いはないとのことでしたが、RVHは比較的移植件数が多く感染症内科が忙しい施設だとのことでした。

 感染症内科の一日は、朝集まり新しいコンサルテーションや前日までのフォローアップの患者さんの担当を確認し、病棟に散っていきます。フォローアップの途中で、新しいコンサルテーションを受けていきます。昼過ぎに外来を終えたattendingのドクター(指導医に相当)を交えてチーム全員で集まり、それまでの報告やディスカッションをします。その後、新規コンサルテーションや問題症例をチーム全体で回ります。
 経験させていただいたのは、敗血症、尿路感染症、脾膿瘍、蜂窩織炎、感染性心内膜炎、肺炎、猫ひっかき病などです。日本での臨床経験がないため単純な比較はできませんが、偽膜性腸炎とその疑いの患者さんは多い印象です。
最も印象に残ったのは、移植前評価のコンサルテーションです。潜在感染症の評価や移植前に勧める予防接種を考えるという経験をできたのは、大変貴重だったと思います。

コンサルテーションという立場のため、多くの病棟に伺う機会に恵まれました。印象としては、救急医療、ICU、移植医療がやはり日本より充実していると思います。特にERは、マンパワー・資材が充実しており、規模や位置づけが日本とは異なると感じました。移植医療も専用の病棟があり、免疫抑制状態での感染管理の難しさを学ぶことができました。それと共に日本での遅れを実感するに至りました。

様々な国から集まってきているので、スタッフも患者さんも様々な背景のため醍醐味があります。実際に、患者さんの出生地や滞在地が全世界に及んでいるので、個々の症例毎に考慮する起因菌も変わっていきます。
オマーンからのレジデントやメキシコからの医学生と一緒に研修し、仲良くなることができました。お互いの国の医療事情や医師としてのキャリアプランの違いを知ることができ、大変刺激になりました。

英語力を不安にされている方もいらっしゃると思います。自分も会話はできますが、少し抽象的なことや、ましてやディスカッションをできる程の英語力を持ち合わせてはいません。そのため、プレゼンテーションが一番の壁でした。他のスタッフがメモ書きでこなせるところでも、プレゼンの下書きや一緒に回っている医学生に対する練習をして十分な準備をして臨むようにしました。

日本が全体的に巣篭もり・内向きになりがちな中でも、オープンマインドさえ持って外に飛び込めば、広い世界と小さな自分を知ることができます。若いうちに多くの違う価値観に触れておくことは損にはならないと思います。

There is like Yoyogi park in Japan. Next to the Olympic stadium, there is huge park.

I've never had a time to read newspaper since the beginning of the last month.
Yesterday it was my first time to buy and read newspaper.

I concerned about the two following articles.

The first one is the title of "Bad medicine."
In the editorial opinion, the author wrote "We have strict rules limiting shift time for pilots and bus drivers; why we are so relaxed about the alertness of medical residents?"
From the side of patients, they want to keep the quality of health care by strict shift time.
How many people know the shift of doctors in Japan?

The second one is "A bonanza for drug makers."
The article point out the problem of DSM (the bible for psychiatry to make a diagnosis).
It made us classified unnecessary psychological diseases. Doctors prescribe more medication. It means drug company earn more.

The quality of articles in newspaper is easily above the Japanese one, which only introduce from other famous newspaper.
Japanese media make so much fuss about an article which called Japanese prime minister "loopy."
Someone said that "the foreign media make us understand essence of the problem, but Japanese media make us get the puzzle"

In my impression, Quebec is under the British influence. They tend to quote British medical journal, like BMJ or Lancet.

I'm looking forward to watch Totem tomorrow. Of course, my seat is the cheapest one.


Yesterday, in my final presentation, I read out "12" as "twenteen." I paid all my attention for the content of the presentation, so I missed to find my mistake of pronunciation.

Fimal weekend in Montreal

I've just decided my schedule in this weekend.

Today, I will visit the Montreal Bontanical Garden.

Tomorrow, I will watch a show of Cirque du Soleil, the title is Totem.
This is my first time to watch Cirque du Soleil. I'm really looking forward to see it!!

終わってみて



ついに終わった。一つの節目だ。

決して1ヶ月楽ではなかった。
1つ1つ壁を登らないとならなかった。
まずは、スーパーの場所、バス・電車の乗り方から学ばなければならなく、PRGで転職した気分になる。

いざ研修が始まると、
何回迷ったかわからず、ページ(PHSも使っているがこちらの方が一般的)の仕方も初めはわからず、チュートを書くのに半日かかったり、枚挙に暇がない。
病棟では無理難題を振っかけられたり、全く関係ない感染症絡みの相談をされたり・・・(イヤイヤ学生ですよ。自分は。そんなこと聞かれても・・・と内心閉口しながらも)
最終日に近づくにつれ、自分の顔を覚えてくれる違う科のスタッフがいたり、他科のドクターやナースとのコミュニケーションもとれるようになってきた。

最初は、語学力不足からトラブルに巻き込まれるのではと、かなりビビっていた。ベッドサイドに行くのが怖くなかったと言えばウソになる。

メキシコの医学生は、以前にも米国でリサーチに参加していて、まだこれから2ヶ月カナダにいる。そして、その後一時本国で実習して、また米国の有名病院2ヶ所に勉強しにいくとのこと。
自分のスケールの小ささにびびった。

それでも、学生にも関わらず、他科のスタッフは真剣に話を聞いてくれる。

ニッコリ笑って「Excuse me?」,「Sorry?」と声をかければ大抵の場合は怪訝な顔はされない。語学力のなさと、人見知りでついつい引っ込み思案になりがちなことも多々あった。が月末に近づくにつれ少しづつ修正されてきた気がする。

今回の研修で、閉じかかっていた自分の目が開いた気がする(まだ完全に開いたわけではないが)。
Open Mindを持ち続けよう。
そして、とりあえず今日は休もう。

day 19


Today was final day to study in Montreal.

Two doctors got their holiday. So today was so hard for staff who remain in the hospital. I followed seven patients and written two new consultation sheets.
First one is in ER. I've had to summarize the history in noisy ER. I've used my energy more than necessity.

This training was clearly divided between on and off. When we start to work, it was busy, but not work so late in languid pace.
Time and tide stay for no man. After start one day, I feel the end of date come in a moment.
We couldn't eat lunch also today. I thought we could eat lunch only half of weekday.

After working, I and another medical student went out to eat late lunch in 6 o'clock p.m. I ate breakfast 6 o'clock a.m.
We told each other about the difference between Wasabi and other spice, national sport in Japan and Mexico, and comics.
He will stay Canada another two months. We are reluctant to leave each other.

I am not afford to consider tomorrow schedule. I've just exhausted.
I've taken a lot of picture, but my energy to upload these today.
