EULAR online course rheumatology 2018 mini-summary

EULAR online course rheumatology 2018にてrheumatology分野をアップデートしています。以下簡単なまとめです。

• 診断思考トレーニング→NEJM CPS読みましょう
• 炎症性筋疾患の鑑別は、Polymyositis、dermatomyositis、Juvenile dermatomyositis、Necrotising myopathy、Inclusion body myositis、Amyopathic / hypomyopathic dermatomyositis、Paraneoplastic myopathy

• Squeeze test単独ではearly RA(発症1年未満)の診断に対する感度低い(腫脹関節の部位やゴールドスタンダードをどれにするかで変わるものの、31-54%) (Ann Rheum Dis. 2015 Oct;74(10):1886-9 PMID:25911456)
• 早期発見し、DMARD開始、その際はMTXが第一選択
• RF高値や抗CCP抗体高値などの予後因子不良なら、MTX+生物学的製剤も考慮
• 非薬物治療としては、禁煙、肥満なら減量
• 極軽症で3ヶ月未満のvery early RAなら、ステロイド筋注単回も考慮される

• 回帰性リウマチの1/3は今後典型的な関節リウマチに進行する
• RFと抗CCP抗体のtiterは関節破壊の度合いと相関する
• RAの精査の一環でANAも測定されがちだが、SLEでRF上昇は稀のため、抗SS-A抗体の方がSjS合併RA検索のために有用（ちなみにSLE＋RAは”rhupus"という）
• 分類不能の関節炎のうち半分はRA(1987診断基準)に移行し、その予測因子は、年齢、女性、関節炎の分布（小関節、全身、上下肢にまたがる）、関節の圧痛と腫脹、朝のこわばりの持続時間、CRP (van der Helm-van Mil et al, 2007)

個人的にはダプトマイシン＋ホスホマイシン静注→MRSA菌血症が衝撃でした。前方向視的研究や他国からの追試にも期待です。

【JAMA neurology original】★Face, Arm, Speech, Time (FAST)キャンペーンを行っても、TIAやminor strokeは救急室利用は変わらず。英国にてFASTキャンペーンを行うと、major strokeは3時間以内の医療機関受診が上昇(OR 2.56)するが、TIAやminor strokeの24時間以内の救急室利用は有意差なし。

【ID week Oral Abstract Session】★★MRSA菌血症に対してダプトマイシン単剤よりホスホマイシン静注した方が、6週間後、7日後の治癒率高い(6週間後42% vs 54%、7日後76% vs 93%)。スペインで行われた4年間で18施設で行われたオープンラベル試験、ダプトマイシン10mg/kg IV群とホスホマイシン2g IV q6hr併用群で1:1に割り付け、副作用は有意差なし。

→TAVIはより安全な方法、適応拡大の方向に進んでいきそうです。昨年JACC interventionにも載っていたデバイスですが、ついに小規模ですがJAMAにも乗りました。新しいデバイスの開発は認可の問題などから、さすがEUが多いですね！！

→先週NEJMに出たliraglutide関連です、DM群では心血管イベントによる死亡と全死亡は下げましたが、HFrEFでは効果乏しいようです

small ovarian follicles →hyper-secreated estradiol → stagnant follicular stage

classic PCOS affect 6-10% of women of reproductive age

heavy menstrual bleeding

① hyperandrogenism, hirsutism 70%

② ovulatory dysfunction→ unpredictable sense < 21 day or > 35 day, but 15-40% of hyperandrogegism has regular mense

③ polycystic ovarian morphologic features: > 12 antral follicles in either ovary, ovarian volume > 10 ml

30-50% insulin resistance, 8-10% DM, 50-80% obese

possibly increased risk of cardiovascular disease

risk of endometrial cancer 2.7 times, 9% of lifetime risk

sub-fertility: pregnancy without medical assistance is common, contraception used as indicated

Dx

①〜③ and exclusion; ex non classic congenital adrenal hyperplasia (early follicular-phase plasma 17-hydroxyprogesterone < 200 ng/dl → r/o 21-hydroxylase deficiency), androgen-secreting ovarian or adrenal tumors

diagnostic criteria is controversial

modified Ferriman-Gallwey score

free testosterone: most sensitive test for hyperandrogenemia with PCOS

androgen assays

Tx

combined estrogen progestin oral contraceptive pills

→ increase hepatic production of sex hormone-binding globulin, decreased free testosterone

30-35μg of ethinyl estradiol is effective managing hirsutism

additional:

spironolactone for hirsutism

episodic or continuous progestin for endometrial protection

metformin for abnormal glucose tolerance; 20-25% lower serum testosterone

clomiphene for ovulation induction

to prevent endometrial hyperplasia, recommend a course of progestin every 1-3 mon

or if contraception is required, daily progestin-only contraceptive pill, or levonorgestrel-releasing IUD

follow-up

screening for cardiometabolic risk: BMI, waist circumference, BP

fasting lipid level every 2yrs

screening for DM with 2-hour oral glucose-tolerance, every 1-5 yrs, depending on characteristics

screening for smoking, OSAS, depression, anxiety
